Hypertrophic cardiomyopathy

Team ClinicSpots replied

30th October, 2019 at 01:01 pm

Hello, HOCM (Hypertrophic obstructive cardiomyopathy) results in abnormal thickening of cardiac muscle. You need to consult a cardiologist. According to your clinical assessment, your treatment will be planned. Investigations like ECG, 2D echo, stress test, Holter monitoring, cardiac MRI, cardiac catheterization may be considered as per the clinical condition. Only then the treatment will be planned like the medical line of treatment to regularize heart rate, heart rhythm, atrial fibrillation and surgical treatments like surgical myomectomy and catheter-based alcohol septal ablation. But the choice of treatment totally depends on the cardiologist decision considering all the involved risk factors. Hope our answer helps you.

So please consult a cardiologist let him re-evaluate the case because for a major surgery many factors are to be considered as the age of the patient, any illness if present, his general condition, weighing benefit over risk and diagnostic evaluation. Once the cardiologist will clinically reassess everything you can decide the further line of action. But along with the treatment lifestyle changes are a must mainly regular and timely food, timely sleep, and distressing. Hope our answer helps you.

Hello Yogendra, Ideally, the stent is considered after 50% blockage. Medical therapy should be considered before the intervention. But risk benefit ratio depends on various factors like your age, presence of any diseases any clinical symptoms and your other reports. Therefore, you must consult an experienced cardiologist and then decide onto further treatment. Hope our answer helps you.

Though cough generally resolves on itself post surgeries, in elderly patients it can sometimes be due to some underlying lung infections, fluid collection. You can always take a second opinion with a cardiologist in this case. Hope our answer helps you.

There are no evidence-based alternative treatments. You need to be in constant touch with a good cardiologist and carry her treatment forward. Hello, In multiple block major arteries, blocks like LAD (Left Anterior Descending) arteries are resolved first. If remaining vessel blockage is not significant clinically it can be handled conservatively. Your father’s chest pain will settle gradually. Consider cardiac rehabilitation post-surgery after consulting your cardiologist. Hope our answer helps you.

I have learned about your institution from my relative. My relative suggested to inform the problem of my wife to your end through mail. My wife Swati Laha aged about 32 yrs was admitted at local hospital at Kolkata for elective LSCS on 14.02.19.At the time of Pre-Anaesthetic check, the doctor advised for ECG,Echocardiography and other allied test.As per ECG report " Right bundle branch block" found and as per Echocardiography report "20 mm ostium secundum atrial septal defect with left to right shunt.Good left ventricular systolic function.Mild pulmonary arterial hypertension." detected. The elective LSCS was done under general anaesthesia on 14.02.19 and female baby delivered. The age of my baby is about two months. In this circumstance,I am in dilemma what to do right now.I am therefore requesting you to kindly guide/ give your valuable suggestion for taking corrective measure of my problem as per ECG/ Echocardiography report.Kindly enlightened me of the following quaries. 1.Is it possible to recover without any surgery. If not, 2.What type of surgery need for 3.Is there need of any kind of test before surgery. 4.What would be the best time for her for surgery as my baby is only 2 months and she was operated on 14.02.19 5.How many days will it be taken to recover after surgery 6.kindly let me know the risk factor of different type of surgery for her problem.

Hello, Looking at your wife's condition we are of the opinion that she has a 20 mm ostium secundum atrial defect. The good news is that having a 20mm ASD is not an emergency case. The treatment can be postponed for 3 to 6 months.
There are basically 2 types of procedures for ASD Closure.
1) Surgical
 2) Device closure
In case of device closure, you can discharge the patient in 2 days and this treatment is done by using a small needle that passes through the groin. In this type of treatment, the patient can resume her daily activities immediately post-treatment.
Surgical closure is done in patients who are unfit for device closure.

 Hope this answers your query.

My wife aged 60 is having slow pumping of blood in the left ventricle after taking ecg, echo and angiogram etc. The functioning of the heart is 65%. As per the cardiologist advice she is taking tablets. It may kindly be advised whether the tablets will speed up the functioning of the heart of otherwise any other treatment I have to undergo. Your advice is earnestly solicited. Thanks

Hello, With the lack of information regarding your wife's condition, prescribed treatment and diagnosis I can only assume what her condition is and what medications has been given to her. Assuming she has low ejection fraction number and the tablets she's been provided are beta-blockers, angiotensin II receptor blockers (ARBs), or Angiotensin converting enzyme (ACE) inhibitors. Low EF number is an early sign of heart failure because of reduced pumping of blood to the rest of the body; these tablets are basic treatment options to prevent it. If you want to consult a different cardiologist I will list a few of the best heart hospitals all over India as I'm unsure of your location. 1. Columbia Asia Hospital, Bangalore 2. Fortis Malar, Chennai 3. Gleneagles Global Health City, Chennai 4. Apollo Health City, Hyderabad 5. Fortis Hospital, Bangalore 6. Asian heart hospital, Mumbai 7. Nanavati Super Speciality Hospital, Mumbai 8. Fortis Escorts Heart Institute, Delhi 9. Indraprastha Apollo Hospital, Delhi 10. All India Institute of Medical Sciences (AIIMS), Delhi 11. Rabindranath Tagore International Institute of Cardiac Sciences, Kolkata

Since you have already begun with your medications there are various other remedies to help with its symptoms such as: 1. Exercise called Valsalva maneuver where you hold breath and strain as if you were trying to have bowel movement, 2. Coughing while sitting with your upper body bent forward. 3. Splashing ice water on your face 4. You also need to avoid smoking, caffeine, alcohol, and illicit drugs. PSVT is not a particularly life threatening disorder. Only if there are heart diseases present, it cause lead to congestive heart failure or angina. There are also many emergency and long term treatments to slow down a patient's heart externally, but those are done only for the patients with repeats episode of PSVT. If you want to consult with different doctors I can recommend some good hospitals. Since I'm not sure of your location and which hospital will be easier for you to visit, I will list some of the best heart hospital in India: 1. Fortis Escorts Heart Institute 2. Indraprastha Apollo Hospital, Delhi 3. All India Institute of Medical Sciences (AIIMS), Delhi 4. Nanavati Super Speciality Hospital, Mumbai 5. Manipal Hospital, Bangalore 6. Fortis Malar, Chennai 7.
