Contraceptive Failure: Chance of Pregnancy?

During intercourse our contraceptive method was broked then I consume unwanted 72 within 1.5 hours after few days i had withdrawal bleeding Is there any chance of pregnancy still?

If patient is on tab itraconazole 200mg od ,if she accidentally become pregnant while taking that tab ,what is the risk to fetus, weather she can continue pregnancy or better to terminate?
