Facial hyperpigmentation may be due to several causes including tan, agespots, melasma, allergy to skin and hair products, association with underlying medical ailments, deficiencies, hormonal imbalance etc. Knowing the underlying cause and diagnosis are essential before the treatment is commenced. Appearance of acne or continuation of acne beyond 25 years of age is called adult acne. Adult acne is most often associated with hormonal imbalance, stress, inappropriate use of skin care products etc.Most common causes include PCOS in women, insulin resistance, certain drugs etc. Treating the underlying cause is important for desirable results. Thorough history, analysis of skin, review of the drugs used, blood investigations may help a dermatologist understand your skin and make a proper diagnosis for satisfactory results. You may also need procedural treatments like salicylic peels, comedone extraction along with topical and oral medications like retinoids, hormonal medications.

Cystic acne at the age 0f 18 years indicates underlying hormonal cause like PCOS, insulin resistance etc. this can be evaluated by certain blood tests and scans. Please consult an experienced dermatologist for the same. Intra lesional triamcinolone injections, oral retinoids, oral contraceptive pills, etc may be recommended once the cause is established by the Dermatologist. Proper dosing and adequate medication course is needed for satisfactory results in severe acne forms like cystic acne.

I'm 19 I have bacterial infection past 2months so I visit to my near general doctor they suggest clonate ointment and candid dusting powder but still there is no improvement?daybyday its increased and itching also I have so I used clobetamil ointment now infection is mildly decreased but this is not permanent solution?so pls give solution for my problem dr

Clonate ointment and candid dusting powder are corticosteroid and antifungal powder respectively which are not recommended for treating bacterial infections and hence it is suggested to stop the same at once. Proper diagnosis and treating the condition accordingly is essential in your case. Ruling out underlying cause and identifying the source of bacterial infections is equally important. Please consult a Dermatologist immediately so that appropriate antibiotics, good skin care regimen and creams can be recommended.

Pigmentation around the eyes may be due to excess melanin in the skin, shadowing effect due to deep set eyes or due to thin skin around the eyes. Pigmentation around the eyes may be due to excess eye muscle strain, inadequate sleep, iron or vitamin B12 deficiencies, thyroid hormonal imbalance or constitutional. Dark circles may extend on to the cheeks leading to pigmentary demarcation lines. However if it is only on one side any trauma or underlying ophthalmologic cause should be ruled out with an Ophthalmologist’s opinion apart from a Dermatologist’s consultation. Topical cream/serum containing vitamin C, retinol, haloxyl, kojic acid , glycolic acid, etc in lower concentrations are recommended. Sunscreens, using sun glasses, anti glare screens on desktop/laptop etc may prevent further worsening of the condition. Laser toning with Q-switched yag laser, mild chemical peels may help. You need a non-comedogenic moisturiser which doesn’t cause any breakout or add to the oily state of the skin. Squalene, ceramide containing moisturisers which do not block the pores are recommended for oily skin. Please consult a dermatologist for thorough analysis of your skin to get a customised prescription suitable for your skin. Each one has a unique skin and has to use best suitable products to maintain a healthy skin. Open pores can be reduced by using retinol containing creams at bedtime. If they are severe procedural treatments like laser toning, micro needling radio frequency can help.

Acne cysts are the most severe form of acne which need immediate attention for they may lead to permanent acne scars. Intralesional corticosteroid injections are given into the acne nodules and cysts are drained for faster resolution of nodules and cysts. Diagnosing and treatment of underlying cause is essential to resolve the acne. Oral retinoids are to be recommended in your case. If hair fall is an issue, Dermatologist may recommend blood tests like serum ferritin, vitamin B12, TSH, vitamin D etc which may be the cause of hair fall. Using the right hair supplements with doctor’s prescription according to the deficiencies, may help treat the hair fall concern. Topical solutions containing Capixyl, Minoxidil etc are also recommended to control hair shedding and promote hair growth.

Itchy hands and feet may be because of eczema, contact dermatitis to detergents, soaps, sanitisers and chemicals, Psoriasis etc. Avoiding triggering factors, excess use of detergents, harsh soaps or sanitisers may reduce the severity and acute flares. Good emollients can help restore the skin barrier. Antibiotics may be needed to prevent infections through the bleeding skin. Mild handwashes and soaps are recommended to prevent worsening of the skin prevent stripping of natural oil from the skin. Oral antihistamines, oral corticosteroids may be recommended for a short duration under supervision in case of severe flares by a dermatologist.

Any hyperpigmentation not responding to creams has to be reviewed. Skin biopsy may be needed to understand the condition and treat accordingly. Sometimes oral medications may also be necessary along with procedural treatments like chemical peels, qsyag laser etc may be needed to get rid of the pigmentation once the diagnosis is established. Acne spots can be treated with depigmenting creams containing ingredients like hydroquinone, kojic acid, glycolic acid, arbutin etc. A good skin care regimen with mild cleanser, moisturiser and a broad spectrum sunscreen are also equally important. Also picking or scratching the acne is to be avoided for it may worsen the spots. Skin creams are to be used as prescribed by a dermatologist who would understand your skin type and recommend accordingly. If the acne spots are severe chemical peels or laser toning may be recommended by the dermatologist.

- Resistant acne and severe acne not responding to conventional treatments needs the consultation of a qualified dermatologist. Most of the times resistant acne has underlying hormonal imbalance which has to be diagnosed and addressed. Certain conditions like PCOS, insulin resistance, steroid abuse, certain drugs may be cause of severe acne. A dermatogist may advise certain blood investigations, to understand the cause behind acne and may prescribe drugs like Oral contraceptive pills, oral retinoids or antibiotics along with procedural treatment to treat the acne and long term resolution
- Genetic tone of skin cannot be changed. However tan or any other acquired skin pigmentation can be improved by topical creams, sunscreens etc. chemical peels, laser toning and other procedures can help in stubborn pigmentation

Post acne marks and post acne scars can be permanent if not treated in time. It is important to treat the ongoing acne and orevent further acne while simultateously treating the post acne marks and scars. Saicylic peels, topical retinoids, comedone extraction are prescribed by the Dermatologists to treat the black heads which are the initial stage of acne. Acne marks can be treated with superficial peels like gycolic acid peels, TCA peels, laser toning etc. Acne scars depending on their type and severity, a stand-alone or combination of treatments including subcision, erbium yag or CO laser, microneedling radofrequencuy or TCA cross etc are used. It is recommended to visit a qualified Dermatologist who would analyse the scars and advise the best possible treatment for scar improvement.

Sun tan is quite common because of unprotected sun exposure. This is because of excess production of melanin or excess accumulation of melanin in the skin layers in response to UV rays. Moles are formed due to arrest of melanin producing cells in skin layers where they continue to produce melanin which collects to form flat or raised moles. Tan can be treated by using certain depigmenting creams which contain glycolic acid, kojicacid, alpha arbutin etc which have to be used under the supervision of a qualified dermatologist. Procedural treatment like chemical peels and laser toning with QS yag laser can help. Most important is religious use of sunscreens to prevent further tan and improvement of skin. Moles can be treated by radiofrequency ablation, punch excision or Q-switched yag laser. So please consult a qualified Dermatologist.

The colour of the skin is determined by the amount of melanin produced in the skin. This in turn is influenced by genetic factors, sun exposure, drugs etc. Uneven skin tone, or any other pigmentation which is acquired and not genetic can be treated with various depigmentation creams which are to be used as per Dermatologist’s prescription. Sunscreens are mandatory to protect the skin from tan and other signs of some damage. Procedural treatments like chemical peels, laser toning etc are recommended apart from topical creams to treat the pigmentary concerns. It is not advised to use OTC creams claiming improvement in skin pigmentation without professional advice. Face washes can never treat pigmentation. Hi. I've a question regarding my niece's skin problem. She is a 7-years old. She has developed red patches of skin on her cheek, chin and around nose. The affected area of her cheek is very dry. I brought her to a doctor who prescribed two creams, mezoderm(betamethasone) and gentamicin-akos which made things far worse. Then at the pharmacy I was advised to use ftorokart(also a cream with triamcinolone) for my niece's face. After a few uses of the cream, I've seen some noticeable improvement in her skin condition since she started to use it. It took the redness out of her nose. But she still has rash and blisters on her face. I took photos of her face in case it might be helpful for you to identify the reason for her skin condition. According to the symptoms and signs described, it appears to be a case of atopic dermatitis which is common among children of the mentioned age. It is a condition where the skin barrier is disturbed and is extremely sensitive to external environmental triggers like cold and dry weather, dust etc. It presents as red dry itchy patches on cheeks, hands and legs sometimes whole body. The above mentioned creams contain topical corticosteroids which are to be used with a great caution under the supervision of Dermatologist. Good barrier repairing creams including emollients with squalene, ceramides etc can help reviving the skin barrier. Steroid sparing drugs can be prescribed to manage the rash. Please consult a qualified Dermatologist and do not use topical corticosteroids without doctor’s advice as it may cause many adverse effects.

Dengue associated rash is quite common and can be a sign of acute stage or resolution stage. Rash may occur within initial two to three days or may occur during the resolution of fever. This may be associated with itch, dryness and peeling of skin The However platelet count has to be monitored during the onset of rash. Supportive treatment like anti histamines and soothing lotions and moisturizing lotions can help treat the rash. Please consult a Dermatologist for appropriate treatment.

Post acne erythema and macular erythematous scars are common in few individuals as the acne subside. Sometimes underlying Rosacea component may also contribute to redness. Oral isotretinoin itself can cause mild erythema as long as the drug is taken if sunscreen is not used appropriately. Quasi long pulse mode of QS yag laser, topical drugs like topical Ivermectin, metronidazole for underlying rosaceaetc are prescribed by the Dermatologists. Please consult a Dermatologist for the same.

Hypertrophic scars are bumpy in nature due to excess scar tissue production and abnormal wound healing post any injury or cut. The choice of treatment would be intralesional Triamcinolone acetonide injections into the scar at 3-4weekly intervals. This may be helpful in reducing the bumpiness of the scar. Dandruff in the scalp is due to excess sebum (natural oil) production along with increased activity of a fungus called Malassezia in the scalp. Antifungal shampoos containing Ketoconazole, Ciclopirox, selenium sulphide can help treat the dandruff. If it is severe, oral antifungals are also prescribed for a short duration. Salicylic acid, coal tar shampoos are also prescribed in case of excess flakiness of scalp. Hair fall may be due to dandruff, nutritional deficiencies, stress or due to genetic factors. It is recommended to visit a dermatologist who can diagnose the cause of hairfall and may suggest treatment accordingly. Trichoscopy of the scalp may help in understanding the nature of the scalp and condition. Nutritional supplements, Capixyl containing serum, Minoxidil solution, oral supplements containing vitamin and minerals are prescribed by the dermatologist to treat the hair loss. Change in hair colour in beard and scalp may be due to nutritional deficiencies or strong hair colours or genetic causes. Dermatologist’s help may be needed to treat the same. Calcium pantothenate containing supplements may help slow down the greying and sometimes restore the colour of hair.
