Hi Abhishek, your injury looks like a ligament or meniscus tear. Need to clinically examine you to diagnose the problem and might require MRI scan to confirm the clinical diagnosis.

First you need to be clinically evaluated for your symptoms and signs. And then a standing knee X-ray to evalute the grade of Osteoarthritis.

Grade 4 Osteoarthritis will require Total Knee Replacement

First reduce the shoulder back to normal position. Give it some rest. 

Most of the times the dislocation will be reccurent and shoulder disloactes/subluxates in abduction external rotation position. 

MRI will be the ideal investigation to asses labral tear and CT scan will be needed to look at glenoid index

ACL partial tear doesn't require surgery. If clinically you have instability in pivoting movements and on examination if ACL is torn (Positive Ant Lachman test), then you will require surgery.

Core decompression is a common procedure done for AVN hip. It can be tried upto Stage 2A. There is nothing wrong in trying core decompression with BMAC. Success rate is pretty decent. If it fails we can always do a Total Hip Replacement any time. 

Current Total Hip Replacement prosthesis have a good longevity. So can keep it as a stable back up option.

At 70 years, Neck of femur fracture needs surgery. Depending on the pre injury activity level we can plan either Total hip Replacement or Hemiarthroplasty (Partial Hip Replacement) 

Its better to operate as early as possible and make sure she is mobilised. Its not advisable to keep her on bed as she can start getting all sorts of complications like bed sores, Chest infections, UTI, DVT, PE Etc
